We believe it is essential to tour a facility to explore how well it meets your needs or those of your loved one. To learn how we can meet your individual needs and to arrange a personal tour, please contact our Admissions Coordinator at (765) 482-6400.
Following admission, a personal care plan will be developed by our interdisciplinary team. This plan will be monitored and modified as necessary throughout your stay, whether long- or short-term. Family and caregivers’ involvement in this process is encouraged through regularly scheduled care plan meetings.
